This new question is unnecessarily confusing and should not be asked. There are two annual visit requirements. All home visits are by definition annual visits and are captured in the monitoring tool through the home contact location. Fieldworker should capture and retain the date of all home visits from the MT and use it to set the next required home visit 365 days from the current home visit date.
The other required annual visit is a non-home visit to a provider which an individual attends for 16 hours or more a week. This is the only new question that needs to be asked and the date this visit occurs should be retained in Fieldworker to set when the next annual visit should occur.
I verified with DDD that DDD and non-DDD providers that an individual attends more than 16 hours a week need to be visited annually by the SC.
I would change the Fieldworker wording to:
Is this a non-home visit to a DDD or non-DDD provider that the individual attends for 16 hours or more a week?
Finally, the non-home visit question should not appear to be part of the MT, but should be asked at the end of the MT.
